AR-15 quick shooting with malfunction

I experience a malfunction during quick shooting drill. The magazine is bad I think. This has never happened before with this weapon. When I pulled the mag, somehow the previously fired casing was stuck backwards into the top of the mag. It's possible I have a sticky extractor, but the other mags worked fine today, and you can see a casing flying away before the malfunction. It's possible I loaded an empty casing in the mag - very unlikely but possible.
