This is the 2nd ending but it's the worst out of the three.  You can get this ending by losing to Chaos in the Chaos Realm.  Keep in mind you can only get to the Chaos Realm if you killed Graham with the Flame Demon, Giant Bat and Succubus Souls equipped.
