- Paciente de 41 años, antecedentes de haber estado con finasteride pero sin tratamiento médico al momento de la consulta. Presenta alopecia difusa con conservación línea frontal, bastante sospechoso de DUPA. Al explorar su donante se constata miniaturización principalmente de zonas temporal y parietal (evidente cuando realizamos un corte de pelo corto).  Medicamos nuevamente con finasteride y explicamos la importancia de continuar tratamiento médico para el resultado a largo plazo. Como siempre decimos las DUPA no son generalmente candidatas a cirugía, pero tienen una posibilidad si están con el tratamiento médico adecuado.

Donante con las siguientes características:

· Temporal: 50 micras de pelo; 63 UF/cm2; 123 pelos/cm2; 1,95 pelos/UF; CV de 6,15

· Parietal: 50 micras; 64 UF/cm2; 143 pelos/cm2; 2,23 pelos/UF; CV de 7,15

· Occipital: 55 micras; 60 UF/cm2; 151 pelos/cm2; 2,52 pelos/UF; CV de 8,30

TDA (área donante total) de 126 cm2 (sólo consideramos la occipital) y TDC (capacidad donante total) de 2327 grafts (a 2,52 pelos/graft).

Se realiza intervención de 1671 grafts (5200 pelos, muy conservadora dada su TDC), tras un año con finasteride,  para restaurar 39 cm2 de frontal (16 a 46 grafts/cm2) y 29 cm2 de coronilla (14 a 21 grafts/cm2) a una media de 2,36 pelos/graft (217 singles; 493 dobles; 622 triples; 302 cuádruples, 36 quíntuples, 1 sextuple). Punch de 0.8/0.9 mm.

This 41 yo patient, was without medical treatment when we first saw him, although he uses to be. He has a diffuse alopecia with a normal frontal hairline, suspect of DUPA. When we assess his donor we recognized hair thinning, mainly in temporal and parietal areas (more noticiable with short hair). We prescribed again finasteride and explain the importance of the medical treatment in the long term outcome. As we always say, DUPA patients are not usually candidates to surgery, but they have a possibility if they are under the right medical treatment.

Donor area with the following characteristics:

· Temporal: 50 microns of hair; 63 FU/cm2; 123 hairs/cm2; 1,95 hairs/FU; CV of 6,15

· Parietal: 50 microns; 64 FU/cm2; 143 hairs/cm2; 2,23 hairs/FU; CV of 7,15

· Occipital: 55 microns; 60 FU/cm2; 151 hairs/cm2; 2,52 hairs/FU; CV of 8,30

TDA (total donor area) of 126 cm2 (we only consider the occipital area) and TDC (total donor capacity) of 2327 grafts (at 2,52 hairs/graft).

We did a 1671 grafts procedure, after one year with finasteride (5200 hairs, very conservative due his TDC) to restore 39 cm2 in frontal third (16 to 46 grafts/cm2) and, 29 cm2 in crown (14 to 21 grafts/cm2) at an average of 2,36 hairs/graft (217 singles; 493 doubles; 622 triples; 302 quadruples, 36 quintuples, 1 sextuple). 0.80/0.9 mm punch.
