Breaker Fire Explodes into Flames for Hot Tub over 300 Volts to 240V Circuit

Here is a case where a service Tech arrived to check out a Hot Tub and Smell something burning when he got out of the service van. He ran to investigate and had taken a ladder with him that day because he knew the Hot Tub was in a high deck. He jumped up and the side of the deck removed the breaker cover and the breaker was smoking. It flamed up and caught on fire just as he turned the breaker off. Measured voltage around the house and it was 158 or so which both legs together adds up to 316 Volts where is should be about 240 Volts or so. We replaced the breaker after the owner had the power company check out the over voltage. We think they replaced the transformer on the pole. Luckily the Hot Tub was not damaged by the voltage.
