The gatekeeper. Titan hard mode is the first time you'll be encountering something hard enough that the riffraff simply won't be able to beat it. It's not the hardest fight in the game by a stretch, but it's the hardest seen yet. I recommend coming into this fight with at bare minimum an average item level of 70. Get better if you can, preferably main body gear. I've shown you how to get i80 body pieces and you can get i90 pieces with mythology tomes. Remember to dodge. If it helps Titan does have a very set pattern as to what he will do when. After the lag patch you don't need to have it memorized or anything, but you've got to be on the ball and know what to do when. Titan drops an item level 80 weapon if you're so inclined as to farm him (do not farm him at average item level 70), but if you follow the relic quest beating him will net you your relic weapon and a few mythology tomes later you'll have your i90 relic zenith weapon.
