Bad transformer. This revision (PCB 13G) had a dual primary/secondary type of 34vct/5vct. The 34v is rectified into +/-16vdc for the op-amps and the 5v goes directly to the VU meter back-lights. I didn't have on hand the correct transformer but I do have a +/- 15vdc and +5vdc. I put this supply in front of the rectifier on the board as it is redundant.
