This is a very unique puzzle that involves Mario directing enemies and items to proper locations. Some enemies follow Mario's vertical position and you can use this to your advantage. Fly Amanita has a prior puzzle called "Button Pushing" that is similar to this, but this expands on the concept quite a bit. The objective in this puzzle is for Big Mario to take a hit in order to become small to access the door to the flagpole. Since it might not be apparent what's going on in the level, the steps are:

1) Direct flying goomba to release a spring, being careful not to have the big mole activate the P-Switch.
2) Bounce the goomba in the boot off the spring and direct into a gap near the muncher on rails, being careful not to have the big mole activate the P-Switch.
3) Have the big mole activate the P-Switch to kill the goomba in the boot, so only the boot will be left. The muncher on rails will push the boot into the clown car.
4) Position the clown car at the same location as the fire piranha plant, so that when the boot expires, the piranha plant will rise and enter the clown car. (this step was the hardest part of the execution for me)
5) Move the clown car all the way to the right so the piranha plant will ignite the big bomb and unblock the two cannons.
6) The cannons will shoot coins which will enter a fire clown car. The fire clown car will ignite two big bombs and a small bomb. The small bomb will blow up a block allowing a cannon to shoot a fish to the right.
7) The fish will traverse the level and enter a fire clown car which will ignite a bomb releasing a ghost.
8) The ghost will damage Mario, allowing Mario to enter the door to the flagpole.
