I did this last semester for a Adobe class project, we had to retouch a damaged photo. I choose a photo of my twin sister and I from the 5th grade. It was damaged in a fire plus had water damaged added on to it, not to mention it wasn't kept neatly at my dad's work shop. 

In the beginning you can see where I already started fixing it before I recorded the screen. I mostly just copy and pasted, and erased areas of the background to make up for damaged areas, flipping and adjusting them to make things look different, and clone stamping places randomly. Blurred the background out to smooth the roughness of it. 

I retouched it using the program Adobe Photoshop CS4. I cleaned it using the clone stamp, cut - copy - and paste, paint brush, blur tool, blending modes, image adjustments and etc. I kept my clone stamp on different opacs and flow, never at 100%. The only time I would ever raise it up would be on skin tones. I tried to speed up the time of this video using windows movie maker, and it kept freezing so I hope it turned out well.
