Sickly Squirrel

I noticed this poor little shivering squirrel walking back and forth on my backyard fence and, upon further examination, saw that it was in really, really bad shape. Scarred nose and all of its fur (except for its tail) had fallen off. What could have happened to it?? Is it suffering from some type of disease? I put fresh water and chunky peanut butter on the fence for it (because I had nothing healthier) but unfortunately other squirrels got to it.
