Dragon Age Origins (PC) - Nightmare Difficulty
Clearing out the first couple of rooms in the Tower of Ishal during the Return to Ostagar DLC quest.  Arcane Warrior/Blood mage is really a wicked powerful combo that makes Nightmare look painfully easy.
