OK folks so this happened to me today.  I have the BRP Tri-Axis handlebars and the adjustment screws came loose over time.  It happened on my ride home too!  They went from not loose to loose!  The video is pretty self explanatory but word of warning to those that have these Tri-Axis handlebars.  CHECK the bolts from time to time!
