Follow the doctor at the beginning of chapter 4 to a house and trigger the cutscene inside. After the cutscene the doctor's brother will immediately attack you. Make sure to draw your weapon quickly or step back a little to get a better shot. Once he's on the ground burn him and the achievement/trophy will pop after the next cutscene.
