Having a congenital heart defect is a lifelong journey. The transition from pediatric to adult cardiology care does not only mean transferring to new doctors. There is so much more to it. Dr. Wayne Franklin, Director of the Adult Congenital Heart Program, discusses how Texas Children's Hospital helps teenagers transition from their pediatric conditions to an adult setting.

6 Tips to Transition from Pediatric to Adult Cardiology Care:

1. Talk to your doctor about when to transition.
2. Take charge of your medical records.
3. Be able to describe your medical condition.
4. Know your medication.
5. Learn how to fill your prescriptions.
6. Know your medical team.

The Adult Congenital Heart Disease Program at Texas Children’s Heart Center enables patients with congenital heart disease to receive seamless continuation of care from birth to adulthood. As pediatric patients with congenital heart defects transition into adulthood, our multidisciplinary team of experienced congenital heart disease specialists advises patients on health and lifestyle choices for their adult needs, including physical challenges, exercise options and family planning.
