A young Norwich Terrier is very itchy 1/2

This Norwich Terrier had ringworm and ear infections some 3 months ago but the itchiness recurs and now the owner wants medication to stop the itch. The owner had an allergy blood test which showed allergies to grass, potatoes, rice, tomatoes and many others. A thick coat with little scales. Paws are licked bald. Ears are OK. Chin is black and bald. What is the solution? Follow-ups by the owner one month after treatment of skin diseases are important but few owners do it.
