This is one of the Schindler Hybrid Man-Powered Elevators, in the Main (Blue, and Green Sections) Parking Garage, at the Holyoke Mall at Ingleside, Holyoke, Massachusetts.
Per usual with parking garages, and malls, this thing looks terrible, but in this case, this MPE actually rides quite well. It has also been tagged a few times. It is of a Hybrid Technology, where it is both a Fixed-Incremented, and Incline Plane MPE.
The other MPE's in the garage complex are exactly the same, so only this one is ridden.
