Loose neck skin could be due to red-eared slider having lost much of his fat or had a fat tumour. The loose skin in the thighs is present too. Dampness and friction led to pressure sores. Sloped plank or dry area is needed for the terrapin to dry out sometimes. No bigger tank and sufficient water for swimming exercise.
